目的:探索体外冲击波治疗脑卒中后下肢痉挛的疗效。方法:计算机检索PubMed、Cochrane Library、
Embase、CNKI、WanFang Data、CBM,搜集体外冲击波治疗脑卒中后下肢痉挛的随机对照试验,检索时限为
建 库 至 2023 年 5 月 。 以 改 良 Ashworth 量 表(modified Ashworth scale,MAS)、Fugl-Meyer 评 定 量 表
(Fugl-Meyer assessment,FMA)、关节活动度(range of motion,ROM)为结局指标,用Cochrane手册进行偏倚
风险评估,采用RevMan 5.4软件进行Meta分析。结果:最终纳入11篇文献,共508例患者。与对照组相
比,干预组患者 MAS(MD=-0.32,95%CI -0.41~-0.23,P<0.05)、FMA(MD=2.05,95%CI 1.32~2.78,
P<0.05)、ROM(MD=1.69,95%CI 0.52~2.86,P<0.05)有显著改善。结论:体外冲击波对脑卒中后下肢痉
挛具有积极作用。
Abstract
To explore the effect of extracorporeal shock wave therapy on lower limb spasticity
after stroke. Methods: A systematic literature review was conducted in PubMed, the Cochrane Library,
Embase, CNKI, WanFang, and CBM databases to identify randomized controlled trials on extracorporeal shock
wave therapy for lower limb spasticity after stroke. The search included all records from database inception until
May 2023. The outcome measures included the modified Ashworth scale (MAS), Fugl-Meyer assessment
(FMA) and range of motion (ROM). The risk of bias was assessed using the Cochrane Handbook and
meta-analysis was performed using RevMan 5.4 statistical software. Results: A total of 11 articles with 508
patients were included. Compared with the control group, the intervention group showed significant
improvements in MAS [MD=-0.32, 95% CI (-0.41, -0.23), P<0.05], FMA [MD=2.05, 95% CI (1.32, 2.78),
P<0.05] and ROM [MD=1.69, 95% CI (0.52, 2.86), P<0.05]. Conclusion: Extracorporeal shock wave therapy
has a positive effect on lower limb spasticity after stroke.
