目的:探讨不同年龄层次的2型糖尿病并发周围神经病变(DPN)患者的危险因素的差异。方法:2
型糖尿病患者178例纳入研究。根据有无并发周围神经病变,分为非DPN组54例和DPN组124例;再根
据年龄差异将124例DPN组分为老年DPN组(年龄≥60岁)和普通DPN组(年龄<60岁);收集各组一般
资料及血液检测结果,并进行统计分析。结果:非DPN组和DPN组患者的年龄、糖尿病病程、糖化血红蛋
白(HbA1c)、2 h餐后血糖(PG)、3 h PG、尿微量白蛋白(ALB)、尿肌酐(UCr)、尿A/C差异有统计学意义(均 P<0.05);Logistic多因素回归分析显示患者的年龄、糖尿病病程是发生DPN的影响因素。非DPN组、普 通DPN组和老年DPN组的年龄、HbA1c、3h PG、Cr、尿A/C差异均有统计学意义(均P<0.05);老年DPN组
和普通DPN组患者的Cr和尿A/C差异均有统计学意义(均P<0.05)。结论:年龄及糖尿病病程是2型糖尿
病并发DPN的影响因素;老年DPN患者出现糖尿病肾病的几率会增大。
Abstract
To investigate the risk factors of diabetic peripheral neuropathy (DPN) in patients with
type 2 diabetes mellitus (T2DM) in different age groups. Methods: A total of 178 patients with T2DM were recruited. Patients were divided into the Non-DPN group (54 cases) and DPN group (124 cases) based on the absence or presence of DPN. The DPN group was further divided into the elderly (≥60 years old) and general (<
60 years old) subgroups according to age. Data were compared between the two groups, and results were statistically analyzed. Results: Between the DPN group and Non-DPN group, the age, course of disease, and levels
of HbA1c, 2hr postprandial glucose (PG), 3hr PG, urinary microalbumin (ALB), urinary creatinine (Cr), and
urinary A/C were significantly different (all P<0.05). Logistic regression analysis showed that age and course
of disease were significantly correlated with DPN occurrence. There were statistically significant differences in
the age, HbA1c, 3hr PG, Cr, and urinary A/C between the Non-DPN, standard DPN, and elderly DPN groups
(all P<0.05). There was a significant difference between the elderly and standard DPN groups in Cr and urinary A/C (both P<0.05). Conclusion: Age and course of disease are the risk factors for DPN occurrence in patients with T2DM. Elderly DPN patients are at an increased risk for developing diabetic kidney disease.
