摘要

儿童青少年双相情感障碍发病率高、症状不典型、易共病其他精神障碍,给临床的诊疗带来了巨大挑 战,并且严重影响学习和社交功能。目前对于这种疾病的具体原因还未完全明确。本文通过综述炎症在儿 童青少年双相情感障碍发病机制和治疗中的作用,发现炎症与犬尿酸通路失调可能是儿童青少年双相情感 障碍的发病机制之一;炎症因子C反应蛋白(C-reactive protein,CRP)和肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)可能是儿童青少年双相情感障碍分期的生物标志物;药物治疗上临床常用的碳酸锂和丙戊酸 类药物可发挥抗炎作用稳定情绪,N-乙酰半胱氨酸、ω -3脂肪酸等抗炎药能够改善儿童青少年双相情感障 碍的抑郁和躁狂,在未来也许可以作为辅助用药助力临床治疗。

Abstract

The high incidence, atypical symptoms and comorbidity with other mental disorders of bipolar disorder in children and adolescents pose great challenges to clinical diagnosis and treatment, and seriously affect their learning and social functioning. The exact cause of this disease is not yet fully understood. This article reviews the role of inflammation in the pathogenesis and treatment of bipolar disorder in children and adolescents, and finds that inflammation and dysregulation of the uric acid pathway may be one of the pathogenesis of bipolar disorder in children and adolescents; Inflammatory factors C-reactive protein (CRP) and tumor necrosis factor-α (TNF-α) may be biomarkers for staging bipolar disorder in children and adolescents; Pharmacological treatments commonly used in clinical practice such as lithium carbonate and valproic acid can stabilize mood by exerting anti-inflammatory effects, and anti-inflammatory drugs such as N-acetylcysteine and ω-3 fatty acids can improve depression and mania in bipolar disorder in children and adolescents, which may serve as adjunctive medications to aid clinical treatment in the future.
