摘要

目的:采用Meta分析评价认知-运动双任务训练(cognitive-motor dual-task training,CMDT)对脑卒中 后患者认知功能的改善效果。方法:计算机检索中国知网(CNKI)、万方、维普、中国生物医学文献数据库 (SinoMed)、PubMed、Web of Science 等国内外数据库(建库至2024年10月),收集CMDT治疗脑卒中后认 知功能障碍的临床随机对照试验(RCT)。使用RevMan 5.4.1进行Meta分析,采用主要结局指标包括简易 精神状态检查量表(MMSE)、蒙特利尔认知评估量表(MoCA)、连线测试 A(TMT-A);次要结局指标为改 良Barthel指数(MBI),评估脑卒中患者经CMDT治疗后认知功能的改善情况。结果:共纳入12篇文献(中 文10篇、英文2篇),总样本量1 032例。Meta分析结果显示,治疗后试验组MMSE与MoCA量表评分均显著 增加(WMD=2.55,95% CI 1.35~3.75,P<0.01;WMD=3.28,95% CI 2.25~4.32,P<0.01)。TMT-A评分显 著降低(WMD=-9.95,95% CI -12.35~-7.55,P<0.05)。试验组MBI量表评分显著增加(WMD=12.58, 95% CI 10.31~14.86,P<0.01)。结论:CMDT对脑卒中后患者认知功能具有改善作用。

Abstract

To evaluate the improvement effect of cognitive-motor dual-task training (CMDT) on cognitive function in post-stroke patients via Meta-analysis. Methods: Domestic and international databases including CNKI, Wanfang, VIP, SinoMed, PubMed, Web of Science (from database establishment to October 2024) were electronically searched to collect randomized controlled trials (RCTs) of CMDT for post-stroke cognitive impairment. RevMan 5.4.1 was used for Meta-analysis. Primary outcome measures included Mini-Mental State Examination (MMSE), Montreal Cognitive Assessment (MoCA), Trail Making Test A (TMT-A); the secondary outcome measure was modified Barthel Index (MBI), to assess the cognitive function improvement of stroke patients after CMDT intervention. Results: A total of 12 literatures (10 in Chinese, 2 in English) were included, with a total sample size of 1032 cases. Meta-analysis showed that the scores of MMSE and MoCA in the experimental group were significantly increased after treatment (WMD=2.55, 95%CI 1.35~ 3.75, P<0.01) (WMD=3.28, 95%CI 2.25~4.32, P<0.01). The TMT-A score was significantly decreased (WMD= - 9.95, 95% CI - 12.35 ~ - 7.55, P<0.05). The MBI score in the experimental group was significantly increased (WMD=12.58, 95% CI 10.31~14.86, P<0.01). Conclusion: CMDT can effectively improve the cognitive function of post-stroke patients.
